Derive FlywayConnectionDetails from JdbcConnectionDetails

Rather than implementing factories for FlywayConnectionDetails for
each supported SQL database, we instead use a factory to takes
JdbcConnectionDetails as a source and produces
FlywayConnectionDetails.
