Add ProblemDetail and @ExceptionHandler support

ProblemDetail is a representation of an RFC 7807 "problem", and this
commits adds support for it in Spring MVC and WebFlux as a return value
from `@ExceptionHandler` methods, optionally wrapped with
ResponseEntity for headers.
