Make Job responsible for setting up step context.  Javadocs on StepContext to indicate that the attributes are volatile.  Add JobParametersAware (might be temporary).

Diagrams have been updated to reflect recent changes; 1) repository has been simplified 2) io-design was simplified to separate ItemReaders design from ItemReaders to simplify DocBook presentation and also reflects the changes to ItemStreamSupport and 3) the domain has been simplified.

Added additional menthod to ItemSkipPolicy to determine if a step should fail. Also added a small prototype of domain-specific interceptors.

BATCH-365: ItemStream#Update and ItemStream#close now accept an ExecutionContext as an argument. I removed the local ExecutionContexts in the readers and readers, and had them use the passed in ones instead.
