Additional fixes for auto-configuration report
Fix the following issues that were introduced in commit 0610378:
- Formatting of code including imports
- Improve hashcode/equals implementations by using ObjectUtils
- Provide hashcode/equals for ConditionOutcome
- Use LinkedHashSet in ConditionAndOutcomes to maintain insert order
